Make an Appointment

Patients are seen by appointment – on the same day the appointment is made if possible. To schedule an appointment, please call the office where you wish to have your appointment scheduled during regular business hours.

Please call in advance for office visits. Make follow-up appointments as you leave. We make every effort to stay on schedule although emergencies do arise. If we are seriously delayed, we attempt to notify patients beforehand.

New Patients

New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.

Insurance & Billing

The Heart Institute participates in most Managed Care Contracts, some of which are listed below:

When arriving for your first visit, please provide proof of insurance. If you are a member of HMO insurance, make sure you receive proper authorization from your primary care physician, prior to arriving for your appointment. We also recommend that you call your insurance company to verify that we participate with your plan.

It is important to notify us if you have a supplemental insurance policy. We accept Medicare assignment with a supplemental policy after both deductibles are met. You are not billed until after the insurance carriers have paid their share of your bills. Then we will send you a descriptive statement of services given and balance due.

If you have any questions regarding the status of your bill, our billing office can be reached at (727) 329-1655 between the hours of 9:00am and 5:00pm. During the first visit, each patient goes through a thorough evaluation including history, physical examination and an EKG.

The Patient Accounts Department is located at the Downtown office. Our Representatives specialize in cardiology coding and billing. They have over 10 years experience in claim filing, Medicare and managed care. They continue to stay updated at all times with new laws pertaining to Cardiology, Electrophysiology and Peripheral issues. Should you need to speak to a Representative in the Billing Department, you may call (727) 329-1655. They will be available for you questions Mon - Fri: 7:30am - 5:00pm.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.

Refill Requests

We are happy to help with refill requests during regular office hours, so please call any of our office locations to let us know what you need.

After-Hours & Emergencies

If you are experiencing a medical emergency, dial 911. However, our physicians are available 24 hours a day in case of emergency. When the office is closed, our answering service will contact the physician on call and you will receive a phone call as soon as possible.
